AWS offers first-trimester abortion services. These services are available by appointment only.

Appointments dates and times are subject to availability.

Detailed information about our services and procedures are outlined in the Information Packet, available for download using the link at right.  Should you decide to schedule an appointment, this is the same packet that you would be required to read and complete on the day of your appointment.  If you are interested in our services, you should read this packet thoroughly, and we encourage you to contact us with any questions you may have.

This is the form required by Indiana State Law that would need to be read, understood, and signed prior to receiving our services.  Note that this must be signed in our presence.  Do not sign prior to arriving for your appointment.

This form lists the associated risks and complications that you must be made aware of, as required by law, before obtaining our services.  Note that this must be signed in our presence.  Do not sign prior to arriving for your appointment.

This form is your legal written consent to allow our physician to perform the procedure as outlined in the packet and as described to you.  Note that this must be signed in our presence.  Do not sign prior to arriving for your appointment.
